Physiotherapy Replacement Clinical ID Card

Monash Clinic ID badges are an essential part of the uniform. Students are required to wear the official Monash photo ID that bears their photo and name when on site visits and during clinical education.

Replacement ID badges are available at a cost of $22 each.

Please bring your proof of purchase (i.e. PG Receipt No.) to the Administration Office, Building G Level 3 - Peninsula campus, for an order to be placed for your replacement ID card
